Liquor License Applications

Applicant Information

All entities applying for an original liquor license must send CB5 an SLA Original Application 30-Day Advance Notice Form, in addition to the CB5 Liquor License Questionnaire, via certified mail/return receipt.

All entities renewing a liquor license must send CB5 an SLA Renewal Application 30-Day Advance Notice Form, via certified mail/return receipt. If additional information or forms are required, the applicant will be notified by the CB5 office.

After an application has been received by the CB5 office, the Public Safety/Quality of Life committee (PSQL) reviews the application to determine if further action needs to be taken. If the committee wishes to speak with the applicant, the applicant will be required to attend the upcoming PSQL meeting, and will be notified accordingly.

The application package must be filled out in full, and the application must be received by the Board Office no later than two weeks prior to the next Public Safety & Quality of Life committee meeting in order for the application to be considered. If the applicant does not hear from the PSQL committee within 30 days of submitting the SLA Advance Notice Form that indicates that CB5 chose not to comment on the application and it goes on record as a silent approval. The applicant can then submit their application to the SLA. A listing of silent approvals for each month can be accessed below.

Failure to fill out materials will result in the Board recommending denial of your application.
