Liquor License Applications
Applicant Information
All entities applying for an original liquor license must send CB5 an SLA Original Application 30-Day Advance Notice Form via certified mail. To verify if a location falls within the boundaries of CB5, please click here.
All entities renewing a liquor license must send CB5 an SLA Renewal Application 30-Day Advance Notice Form, via certified mail/return receipt. If additional information is needed, the applicant will be notified by the CB5 office, and asked to submit a CB5 Liquor License Questionnaire.
After an application has been received by the CB5 office, the Public Safety/Quality of Life committee (PSQL) reviews the application to determine if further action is needed. If the committee wishes to speak with the applicant, the applicant will be asked to attend the upcoming PSQL meeting, and will be notified accordingly.
If the applicant does not hear from the PSQL committee within 30 days of submitting the SLA Advance Notice Form,this indicates that the committee has chosen not to comment on the application. The applicant can then submit their application to the SLA.
The application package must be filled out in full, and the application must be received by the Board Office no later than two weeks prior to the next Public Safety & Quality of Life committee meeting for consideration in the upcoming meeting cycle. Failure to fill out materials will result in the Board recommending denial of your application.
